Custom Milled Implant Bar Cases

Custom Milled Implant BarMilled implant bars are becoming a more popular option with the older dental patient who is looking for a cosmetic as well as a functional replacement for their old denture. The ability of the milled over denture bar that can be screwed to place and will not flop around, cause sore spots, and allows the patient to eat a wider variety of food, not only helps the patients overall health, but also can help to restore some self esteem that may have been lacking because of an unsightly  smile.

Milled implant bars can be a great option for your practice and a good money maker if a cookie cutter approach is taken with the case.

Some of you may be intimidated to try this kind of custom milled implant work, but with the help of the team here at Las Vegas Esthetics Dental Laboratory we can help you through your first cases one step at a time.

One of the best things you can do right off the bat is to let the patient know that the process from start to final delivery of the milled implant bar is going to take a while. They will be impatient; they just waited 6 months for the implants to heal now they want their teeth, like yesterday!!

Let your patient know that you will be seeing them at least 6 times for various try in and quality checks. If you can prepare the patient beforehand then the process is much easier for you and your team.

Las Vegas Esthetics Dental Laboratory will also provide a cost estimate to you upon request. This estimate will give you a solid platform to form a quote for the patient; you can then give the patient a realistic view of the final cost of the custom milled implant bar.

Here is an outline of how may appointments will be needed to complete the custom milled implant bar.

1. The first appointment is to take preliminary impressions for the custom trays to be fabricated. (2 days in lab)

2. The next appointment will be for the master impression, place impression pieces into implants, and treat it like a denture impression. Be sure to capture all tissue land marks and the implants. Take a new opposing also.

Lab will make base plate and bite block and return to Doctor for bite. (3 days in lab)

3. Doctor then takes a bite with bite block marking midline and lip line. Send to lab.

Lab will mount case and do setup for try in. Lab will also make a Model verification jig for try in. (7 days in lab)

4. Doctor to try in the setup check esthetics and function get patient approval. Also try in the model jig during this appointment.  Send back to lab.

Lab to fabricate the milled bar and process the case to finish. (15 days in lab)

It has been found that over 70% of your patients have periodic or regular headaches…  did you know that YOU can help them treat this with a simple Orthotic?

(Here are the components needed to insure the best possible Neuromuscular Orthotic for your patients success…)

It is very important to take current upper and lower poly vinyl impressions. Check for clarity or distortion especially in the lower impression for a precise fit.  Several models are needed in this process.

If possible pour models as a check for Shimbashi and  mark customarily on 3-30/8-25/14-9 for the CO and MYO bite.  If you are unable to pour models then record the anterior vertical on RX.

Check your MYO bite – is it at least 2mm for strength and adjusting? It can easily lose .5mm of vertical in adjusting.  If you hold MYO bite to the light and can see through it then you are at .5mm from the CO.  If this is for restoration and you are less than 2mm for G.V. then a fixed orthotic is your best choice.  For pain… at least a 3-4mm bite is recommended.

A MYO bite placed on models will show the anterior or posterior shift.  Afterward return MYO bite to patients mouth and ask them the most important question… “How does that feel?”

These are a few ways to double check and understand how successful your myo bite to neuromuscular orthotic will be to your patient.
